To receive the contracted rate for services from a provider, you must pay the amount due at the time of service.  The billing department will call the telephone number on your Empyrean card to verify your Membership and access the contracted rate for services rendered.  You are responsible for paying that amount at that time.

The exception to this may be hospitals* or other medical facilities.  You must contact Empyrean's Member Services to notify us prior to going to the hospital so that we can negotiate the rate and have the admittance approved.  In the event of an emergency, of course, go to the nearest facility to seek treatment.  Please notify Empyrean within 48 hours so we can negotiate rates on your behalf.
