What are the deductibles and co-pays with Empyrean plans?

It depends on the plan you choose. Major medical plan co-pays and deductibles vary. There are no deductibles with limited medical plans as they reimburse the first dollar of covered expenses and up to the plan maximum for our member AND all eligible dependents!
